Resumen: | Medir el riesgo anual de infección tuberculosa (RaIT) y estimar su tendencia evolutiva en escolares del distrito de Trujillo y El Porvenir. Material y métodos: Se realizó un estudio observacional, transversal, prospectivo y randomizado, en 1324 niños de 6 a 9 años de edad, no vacunados con BCG de 34 escuelas de Trujillo y 16 de El Porvenir en 1987 y repetida en 920 escolares de la misma edad, escuelas, distritos y equipo técnico en 1993. Atodos los niños, se les aplicó 0.1 ml de PPD intracutánea. Se midió la induración 48-72 horas después y se consideró positiva aquella con diámetro > 10mm. Resultados: Primera encuesta: En Trujillo la prevalencia de infección tuberculosa (PIT) fue de 6.3 por ciento y el RAIT de 0.93 por ciento. En El Porvenir, la PIT fue 9.5 por ciento y RAIT 1.27 por ciento. Segunda encuesta: En Trujillo, la PIT fue de 5.5 por ciento y RAIT 0.78 por ciento, mientras que en El Porvenir, la PIT fue 6.4 por ciento y RAIT 0.81 por ciento. Se estimó la disminución anual de RAIT en 3 por ciento para Trujillo y 7 por ciento para El Porvenir. Así, Trujillo disminuirá el 50 por ciento de su RAIT en 20 años (declinación lenta); El Porvenir en 10 años (declinación rápida). Conclusiones: La PIT y la RAIT han disminuído desde 1987 a 1993, pero permanecen altos, la declinación anual del RAIT es rápida en El Porvenir y lenta en Trujillo. Se estima que el problema de la tuberculosis persistirá más allá del año 2003. Material and methods: An observational, transversal, randomized and prospective study was made in 1324 children from 6 to 9 years old, both sexes of 34 schools, districts and technical team in 1993. All children received 0.1 ml of PPD intracutaneous. The induration was measured 48-72 hours later and considered positive if the diameter was>10 mm. Results: First study: The prevalence of tuberculous infection (PTI) was 6.3 per cent and ARTI 0.93 per cent in the district of Trujillo while in El Porvenir PTI was 9.5 per cent and ARTI 1.27 per cent. Second study: PTI was 5.5 per cent and ARTI 0.78 per cent in the district of Trujillo while in El Porvenir PTI was 6.4 per cent and ARTI 0.81 per cent. The annual disminution of ARTI was estimate in 3 per cent for Trujillo and 7 per cent for El Porvenir. Thus, Trujillo will decrease 50 per cent of its ARTI in 20 years (slow decline), in El Porvenir will ocurr in 10 years (quick decline). Conclusions: The PTI and ARTI were decrease since 1987 to 1993 bur remain high. The annual decrease is quick decline in El Porvenir and slow decline in Trujillo. The tuberculosis problem will persist further the year 2003. (AU)^ien.
