Resumen: | OBJETIVOS: El propósito fue determinar el valor diagnóstico de la angiotomografía computada multidetector en el diagnóstico de aneurismas cerebrales comparado con la angiografía por sustracción digital y determinar el índice de complicaciones con ambos métodos diagnósticos. MATERIAL Y METODOS: Estudio descriptivo retrospectivo de los registros de informes de 84 pacientes (56 mujeres, 28 hombres; rango de edad 24-84 años) con sospecha clínica de aneurisma cerebral a quienes se les realizó angiotomografía computada multidetector y angiografía por sustracción digital en el Hospital Nacional Edgardo Rebagliati Martins, entre enero y diciembre de 2011, fueron incluídos en el estudio y los resultados fueron comparados. RESULTADOS: 72 aneurismas fueron detectados con angiografía por sustracción digital en 56 pacientes, en 28 pacientes el resultado fue negativo. Comparado con la angiografía por sustracción digital, en relación al número de pacientes, para la angiotomografía computada multidetector, se obtuvo una sensibilidad global de 83.9 por ciento, especificidad de 92.8 por ciento, precisión diagnóstica de 95.9 por ciento, valor predictivo positivo de 95.2 por ciento y valor predictivo negativo de 74.2 por ciento. Sin embargo, para aneurismas <3 mm, tuvo una sensibilidad de 30 por ciento, especificidad de 92.9 por ciento, precisión diagnóstica de 76.3 por ciento, valor predictivo positivo de 60 por ciento y valor predictivo negativo de 78.8 por ciento. Además, la angiotomografía computada multidetector fue también precisa en determinar su localización. La tasa de complicaciones fue tres veces mayor con la angiografía por sustracción digital sin diferencia significativa entre los dos métodos diagnósticos. CONCLUSIÓN: La angiotomografía computada multidetector tiene una alta sensibilidad en detectar aneurismas y un alto valor predictivo positivo (especialmente con aneurismas >3 mm). (AU)^iesPURPOSE: Our purpose was to determine the accuracy of multidetector CT angiography in detecting cerebral aneurysms compared with digital subtraction angiography and to estimate the complication rate of both assessment methods. MATERIALS AND METHODS: A descriptive retrospective study from the collected data of 84 consecutive patients (56 women, 28 men; age range 24-84 years) with suspected cerebral aneurysms who had undergone both MDCT angiography and OSA in the Edgardo Rebagliati Martins National Hospital, between January to December 2011, were included in the study and the results were compared. RE5ULTS: 72 aneurysms were detected at digital subtraction angiography in 56 patients, whereas no aneurysms were detected in 28 patients. Compared with digital subtraction angiography, on a per-pacient basis and for multidetector computed tomography angiography the overall sensitivity was 83.9 per cent, the specificity was 92.8 per cent, the accuracy was 95.9 per cent, the positive predictive value was 95.2 per cent, and the negative predictive value was 74.2 per cent. For aneurysms of <3 mm, however, had a sensitivity of 30 per cent, specificity of 92.9 per cent, accuracy of 76.3 per cent, positive predictive value of 60 per cent, and negative predictive value of 78.8 per cent. In addition, multidetector computed tomography angiography was also accurate in determining its location. Complication rate was three times more for digital subtraction angiography than multidetector computed tomography angiography, however the difference between both assessment methods was no significant. CONCLUSION: Multidetector computed tomography angiography has a high sensitivity in detecting aneurysms and a high positive predictive value (especially aneurysms >3 mm).
