Resumen: | Objetivo: Determinar el punto de corte de ADA, su sensibilidad y especificidad y los valores predictivos positivos y negativos, que presentan los pacientes con diagnóstico de tuberculosis pleural del Policlínico Peruano Japonés entre diciembre 2009 y junio 2014. Metodología: El presente trabajo es un estudio observacional, analítico, de corte transversal; donde se revisaron las historias clínicas del Policlínico Peruano Japonés entre diciembre 2009 y junio 2014. Para determinar el punto de corte del Test de ADA se realizó el análisis de curvas ROC y se determinó el punto donde se obtuvo la más alta sensibilidad, especificidad, valores predictivos positivos y negativos. Resultados: Los valores promedio de ADA en el grupo con biopsia positiva fue de 89.5 UI/L y de 51.1 U/L para el grupo de biopsia negativo (p<0.05). Además la edad media registrada según los pacientes con biopsia positiva y negativa fue de 33.3 y 65.5 años respectivamente (p<0.05). A su vez con respecto a los resultados de la biopsia, poco más de la mitad, correspondió al diagnóstico de TBC pleural (57.9 por ciento). El punto de corte de ADA 61 UI/L, teniendo una sensibilidad y especificidad de 95.5 por ciento y 73.3 por ciento respectivamente; adicionalmente, un valor predictivo positivo de 84 por ciento y un valor predictivo negativo de 91.6 por ciento, con un área bajo la curva de 0.806 y una eficacia de 86.4 por ciento. Conclusiones: Se concluye que la primera causa de derrame pleural encontrada fue tuberculosis pleural (57.9 por ciento), el sexo predominante fue el masculino (59.1 por ciento) y la edad media fue de 33.3 años. Se sugiere como punto de corte de ADA el valor de 61 UI/L para la población estudiada. In addition the average age recorded as patients with positive and negative biopsy was 33.3 and 65.5 years, respectively (p<0.05). In biopsy results, just over half corresponded to diagnosis of pleural tuberculosis (57.9 per cent). The ADA cutoff of 61 IU/L, with a sensitivity and specificity of 95.5 per cent and 73.3 per cent respectively; additionally, a positive predictive value of 84 per cent and a negative predictive value of 91.6 per cent, with an area under the curve of 0.806 and an efficiency of 86.4 per cent. Conclusions: In conclusion the main cause of pleural effusion found was pleural tuberculosis (57.9 per cent), the majority of patients were male (59.1 per cent) and the mean age was 33.3 years. Suggested as ADA cutoff value of 61 IU/L for the population studied. (AU)^ien.
